How to create and use the Survey card
The survey card is available in the editor modal card picker. Creators can create a survey as part of any self-paced course (Note: surveys are not available in live content) and add as many of the available question types as wanted.
Options
Allow anonymous submissions: If toggled on, this allows the respondent of the survey to select if they want to submit their response anonymously or not.*
*If the user restarts the course, they can submit their answers again, but their previous responses will still be visible in the response data.
Mandatory question: each question can be marked as mandatory, and the respondent must then answer the question to be able to submit the survey. If no questions are mandatory, the respondent must still submit the survey (even if no questions are answered) to complete the survey as part of the course.
Disable regular course feedback: from course settings, the default course feedback that is shown at the end of the course can be disabled, if the survey card is used as custom feedback.

Responses
Responses for a survey can be seen and downloaded from the create modal of the course.
Caution: once responses are collected, updating the survey questions may affect data integrity. Changing questions will not create a new version, and deleting questions will delete all the responses as well.
FAQ
What happens to the data if the survey is changed after responses have already been collected?
A warning will pop up to warn the user before they make any changes if the survey already has responses. But the following changes to the survey will result in various scenarios:
deleting a question will delete the data associated with that question
deleting an option will delete the option data from responses (the option text/ scale number)
changing the anonymous setting will not update the anonymous/identifiable of previous responses
updating questions will update the response data too, so no data is stored regarding the actual formulation that was used when the learner responded to the question
